This program provides you with transportation and assisted transportation services if you have no other way to get to your appointments. You can use the transportation services to:

  • Go to appointments with your health care provider or dentist
  • Pick up prescription medications
  • Attend meetings concerning Social Security, Medicare, and legal services

The program also provides outreach services, information, and referrals.
