This program provides you with door-to-door transportation services if you have a disability and can’t take public transportation. For example, you may not be able to travel alone on public transportation or may need a wheelchair lift that isn’t available where you want to board.

The program only provides transportation to places within a 5-mile radius of your home. You will need to reserve a ride in advance and pay a fare for each trip. A personal care assistant can ride with you for free.

To apply for this service, please print and fill out the application form and send it back to the program.
