This program gives you health care and daytime support services Monday through Friday so that you can stay in your own home to avoid having to move into a nursing home. This program can help you take care of your daily needs by providing the following services:

  • Nursing and personal care: You can have help taking your pills and help with grooming, toileting and other personal needs.
  • Physical rehabilitation: You may participate in exercise programs that will strengthen your muscles and help you move.
  • Meals and snacks: Lunch is served daily.
  • Social, educational and recreational activities: All kinds of entertainment that you can enjoy with other people in the program.
  • Transportation: You will be picked up in the morning and dropped back at home at the end of the day.
