If you are having trouble taking care of yourself on a daily basis, this program will provide assistance so you can remain independent. You will receive supportive services allowing you to live in a community setting with a caregiver. The services may include personal care assistance, such as help with bathing and dressing, help with household tasks, and case management.


How do I apply?

To apply for services from the Adult Family Care program, contact your Aging Services Access Point (ASAP) office.
