This program helps you find resources if you, or someone you care for, has Alzheimer’s disease or another form of dementia. You can speak to a professional to find services where you live, or call a helpline at any time. Services can include:

  • Home care
  • Adult day care
  • Help finding caregivers
  • New homes built for older adults and caregivers
  • Lawyers who work with older adults
  • Transportation

If you are a caregiver, you can get help finding resources on Alzheimer’s disease and other types of dementia. You can also learn about a special program to help find people who wander because of Alzheimer’s or another type of dementia.

How do I apply?
