This program gives you health care and support services so that you can stay in your own home and stay independent. This program can help you take care of your everyday needs with the following services:

  • Case management: A social service professional will work with you and your family to decide which services you will need so that you can stay at home.
  • Homemaking: This helps with housework, laundry, shopping, and running errands.
  • Personal care: A service professional will help you with your daily activities such as bathing, dressing, and grooming.
  • Meals: This gives you home-delivered meals and helps you with making meals at home.
  • Adult day programs: You can take part in social activities outside of your home. Adult day programs may also include health care services.
  • Adult family care: If you have a caretaker that you live with, they may get a cash benefit to help you with your daily living needs.
  • Respite care: These are programs and services for your caregivers to give them a break.
  • Personal emergency response system: This service helps you if there is an emergency. For example, if you have fallen and cannot get up or you need other medical, fire, or police help.
