This program gives you health care and support services so that you can safely stay in your own home. You can get help from the following services:

  • Case management: A social service professional will work with you and your family to decide which services you will need so that you can stay at home.
  • Homemaking: This includes help with housework, laundry, shopping, and running errands.
  • Personal care: A service professional will help you with your daily activities such as bathing, dressing, and grooming.
  • Meals: You can get home-delivered meals and help with preparing meals at home.
  • Adult day programs: You can take part in social activities outside of your home. Adult day programs may also include health care services.
  • Transportation: You can get rides to doctor appointments and other important errands.
  • Chore service: You can have help with heavy housework, yard work, or taking care of your sidewalks.
