Family Care Program
This program provides long-term care services if you need help taking care of yourself. The Family Care Partnership Program combines health care and long-term care services like help at home and meal delivery. In many cases, you are able to get the services you need to remain in your own home. The program is available throughout most of the state but is not in every county.
