This program helps you learn to select and hire your own care provider (called a home care worker). A home care worker can be a friend, neighbor, or relative who can provide you with services such as: housework, laundry, bathing, getting dressed, cooking or getting meals, and other health services.

A case manager will help you arrange the services you need to select a trained provider. The case manager will also help you understand the services you can get and help to identify the things you will need to make your home safe for you to continue to live in.
