Social Services to Adults (SSTA)
This program helps you to continue living at home by providing the following services:
- Case Management: A social worker will work with you to complete an assessment of your needs and resources and will come up with a plan of service to help you to continue living independently in your community. If it is determined that may need institutional care, the program will help you find a place.
- Information and Referral: You will be referred to another agency if the program determines that your needs are best served by another agency.
- Crisis Intervention: This is a 60-day service that will help you during a crisis or provides you guidance through a change in your life.
