This program can help you continue living at home instead of moving to a nursing home. You can get health care and support services to take care of your daily needs with the following services:

  • Adult day service: You can take part in social activities that occur outside of your home. Adult day care may also include health care services.
  • Attendant care: A service professional will help you with your daily personal care activities, such as bathing, dressing, and grooming.
  • Case management: A social service professional will work with you and your family to decide which services you will need to remain living at home.
  • Homemaking: This includes services such as housework, laundry, shopping, and running errands.
  • Meals: This provides you with home-delivered meals and helps you with preparing meals at home.
  • Respite care: These are programs and services provided to your caregivers to give them breaks.
  • Personal emergency response system: This service provides you with help in the case of an emergency, such as if you have fallen and cannot get up or if you need medical, fire or police help.
