This program offers you primary health care services in suburban Cook County and parts of Chicago. By taking part in this program, you will be able to get treated by a primary care doctor for small co-payment of $5 per visit or procedure. The program also offers prescription medications for a small copayment of $10 for generic drugs, $20 for preferred brand name, and $30 for non-preferred brand name prescription drugs. The program has an annual fee based on your family size of: $20 (for one), $40 (for two), and $50 (for three or more).
