This program provides services at their centers if you need help taking care of yourself during the day due to a chronic condition. Some of the services you can take part in include social activities, having your health monitored, getting physical therapy and learning about how to take better care of yourself. The program also offers meals and transportation to and from the center.

If you are caring for a loved one with a chronic condition, the Center can provide you with education, support and respite care. A case manager can work with you to select services for the person you are caring for. You can also take part in support groups and one-on-one sessions with a professional.
