Connecticut Home Care Program for Elders (CHCPE)
1 min read
Share

This program helps you so that you can continue living at home instead of moving into a nursing home. You will get help with activities of daily living such as: cleaning, cooking, and other light housekeeping. The CHCPE can also help you with some medical needs by providing a visiting nurse or home health aide.

There are two parts to the program: CHCPE State-Funded and the CHCPE Medicaid Waiver. Depending on which part of the program you qualify for, you may have to pay a cost for the services and there may be a waiting list.

How do I apply?

You can apply for this program by printing and filling out the application. If you have questions or need help applying, please contact the Alternate Care Unit at: (800) 445-5394 (in-state only) or (860) 424-4904. You can also contact your local office.
