This program gives you health care and support services so that you can safely stay in your own home. You can get help from the following services:

  • Case management: A social service professional will work with you and your family to decide which services you will need so that you can stay at home.
  • Homemaking: This includes help with housework, laundry, shopping, and running errands.
  • Personal care: A service professional will help you with your daily activities such as bathing, dressing, and grooming.
  • Meals: You can get home-delivered meals and help making meals at home.
  • Adult day programs: You can take part in social activities outside of your home. Adult day programs may also include health care services.
  • Adult family care: If you have a caretaker that you live with, they may get a cash benefit to help you with your daily living needs.
  • Respite care: These are programs and services for your caregivers to give them a break.
  • Personal emergency response system: With this service you can get help if there is an emergency. For example, if you have fallen and cannot get up or you need other medical, fire, or police help.
