This program gives you help so that you can stay in your own home. You can take part in a local day program with social activities that take place outside of your home. The person who takes care of you can get respite services that help when they need relief. The services can include:
- Case management: A social service professional will work with you and your family to decide which services you will need so that you can stay at home.
- Homemaking: This includes help with housework, laundry, shopping, and running errands.
- Personal care: A service professional will help you with your daily activities such as bathing, dressing, and grooming.
- Meals: This gives you with home-delivered meals and helps you with making meals at home.
