This program can help you continue living at home instead of moving to a nursing home. It provides you with health care and support services to take care of your daily needs by providing the following services:

  • Case management: A social service professional will work with you and your family to decide which services you need to remain living at home.
  • Personal care: A service professional will help you with your daily personal care activities, such as bathing, dressing, and grooming.
  • Chore services: This provides you with help for home-related chores, such as heavy housework or yard work.
  • Homemaker services: This includes services to help you with daily living tasks, such as housework, laundry, shopping, and meal preparation.
  • Respite care: These are programs and services provided to your caregivers to give them breaks.
  • Personal emergency response system: This service provides you with help in the case of an emergency, such as if you have fallen and cannot get up or if you need medical, fire, or police help.
  • Adult day care: This service lets you take part in social activities that occur outside of your home. Adult day care may also include some health care services.
  • Hospice: This service provides you with assistance, so you can stay at home if you have a terminal illness.
