If you have a disability and are working, this program may provide you with health care benefits. In some situations, working and earning money could prevent you from getting public benefit programs such as Medicaid.

This program allows you to work and still keep your health care coverage. However, you may have to pay a fee. The amount you pay may depend on your income (called a sliding scale fee). The premium amount cannot be more than 7.5% of your total income.


How do I apply?

To apply for this program, you can do one of the following:

  • Use the online application. On the last page of the online form, you should put “Application for HWD” in the comments section.
  • Print and fill out the paper application and mail it to the Department of Social and Health Services (also called DSHS) Hub Imaging Unit in Tacoma, Washington.
  • Call and complete an application with the Specialized Medical Team at 1-206-272-2169 or toll-free at 1-800-871-9275.
