This program provides several options for you to get help if you need personal care. In some cases, services may help you remain living at home with some support. You can also get an assessment if you are going to move into assisted living. Other services may include:

  • Home-based services, such as companion care or chore and homemaking services
  • Adult foster care
  • Adult day care
  • Nutritional counseling and meal preparation
  • Screenings for those entering nursing facilities
  • Social activities for you to participate in
