This program can help you continue living at home instead of moving to a nursing home. The program provides you with health care and support services to help take care of your daily needs by providing some of the following services:
- Case management: A social service professional will work with you and your family to decide which services you need in order to stay at home.
- Personal care: A service professional will help you with daily personal care activities, such as bathing, dressing, and grooming.
- Homemaker services: This includes services to help with regular household tasks, such as housecleaning, laundry, shopping, and preparing meals.
