This program helps you learn to select and hire your own care provider (also called a home care worker). A home care worker can be a friend, neighbor, or relative who can provide you with care services, such as housework, laundry, bathing, getting dressed, cooking or getting meals, and health services.

A case manager will help you arrange the services you need to select a trained provider. The case manager will also help you understand the services you can get and help identify the things you will need to make it safe fto continue living at home.
