This program helps you to continue living at home by providing the following services:

  • Case management: A social worker will work with you to make an assessment of your needs and resources and will come up with a plan of service that will allow you to continue living independently. If it is determined that you may need institutional care, the program will help you find a place.
  • Information and referral: You will be referred to another agency if the program determines that your needs are best served by another agency.
  • Crisis intervention: This is a 60-day service that can help you during a crisis or provide guidance through a change in your life.
