Hart-Supported Living Program

Hart-Supported Living is a program based on individually designed plans for support. These plans provide services that help you live successfully in a home of your choice and participate in your community. You help create your own plan and pick services that meet your needs. Services can include:

  • One-time support: This provides adaptive and therapeutic equipment, such as TTY/TTD modules, service animals, and assistive technology. It also provides home modifications and vehicle modifications.
  • Ongoing support:  This provides attendant care services, homemaker services, respite care, live-in support, transportation, and a trainer who provides home management and independent living skills.

How do I apply?

To apply for this program, you can print and fill out the application form and mail it to the Regional Hart-Supported Living Coordinator by April 1.
