The Northern Kentucky Adult Dental Assistance Program helps adults living in Boone, Campbell, and Kenton counties get dental care at no cost. The program connects eligible adults with dentists for important services like exams, cleanings, fillings, and more.

  • Dentures or partials

There may be a waiting list for treatment based on the number of applications received in your county.

Am I eligible for the Northern Kentucky Adult Dental Assistance Program?

You can get help from the Adult Dental Assistance Program if you meet all of these requirements:

  • You live in Boone, Campbell, or Kenton County, Kentucky
  • You have no dental insurance or very limited dental coverage
  • Your household income is at or below the monthly limit for your family size

How do I apply for the Northern Kentucky Adult Dental Assistance Program?

To apply for the Northern Kentucky Adult Dental Assistance Program, you will need to complete an application and sign a Consent and Rules Form. You can complete the application online or download and print paper versions by clicking below:

When applying, you will need to include the following required documents:

  • Copy of your photo ID
  • Proof of total household income (such as pay stubs or income tax return)
  • Proof that you have lived in your county for at least 60 days (such as a utility bill or other mail showing your address)

Once completed, submit your application and copies of required documents to the NKY Health program coordinator. You can submit by fax at 859-578-3689 or drop them off at the Boone, Campbell, or Kenton County Health Center or at the NKY Health District Office. Find a location near you.

After your application is reviewed, the program coordinator will contact you to let you know if you are approved and help you choose a dentist in the program.
