This program provides you with services to help you continue living at home and avoid moving to a nursing home. Services may include:

  • Adult day service: You can take part in social activities that occur outside of your home. Adult day care may also include health care services.
  • Case management: A social service professional will work with you and your family to decide which services you will need to remain living at home.
  • Emergency home response system: This service provides you with help in the case of an emergency, such as if you have fallen and cannot get up or if you need medical, fire, or police help.
  • In-home service: This includes services such as housework, laundry, shopping, and running errands.
