Case Management Program for Frail Elders

This program provides a case manager who will help you create a plan of care. A plan of care can help you get services that are right for your needs, so you can continue living at home instead of moving to a nursing home. Services may include:

  • Home delivered meals that are based on your nutritional needs
  • An emergency response system that helps you in the case of an emergency, such as if you have fallen and cannot get up or if you need medical, fire, or police help
  • Nursing and homemaker services to help with your medication, dressing, and bathing
  • Chore services to do your housework
  • Assistive devices that can help you with your hearing or walking
  • Adult day services and respite care that provide you with activities you can take part in during the day to give your caregiver a break
  • Transportation services to help you get to and from your doctor appointments or adult day center
